Policy

Students are required to obtain or provide documentation of a NPI number during their first year in the program. Students must keep their NPI number on file with the Office of Clinical Training (OCT).

Procedures

Please refer to the National Plan and Provider Enumeration System (NPPES) website for application details. To apply, students must create an account on the NPPES website and click on the "Apply for an NPI for myself Button." Be aware that the NPI number will be students' NPI for their careers. Students may modify their information at any time in the NPI system.

It is recommended that students submit their application for an NPI number by October 15th of their first year in the program, as it may take some time to process.

Students are required to submit their NPI number to the OCT by December 15th of their first year in the program. You are not able to participate in the practicum placement process until the NPI number is on file with the OCT.
